To maintain our current Licenses we are required to establish and maintain very detailed child files. Each child enrolled in the program receives a packet of forms which must be returned and processed
before they can attend the Extended Day Program. Copies of these
forms are available to download. Completion of these forms DOES
NOT mean your child is enrolled. You must receive confirmation
from the Director. |
